Additional Items Needed for Registration

·        Copy of Birth Certificate     
·        Player Signature
·        1x1 Picture of Player
Spring 2012 registration forms will be accepted through January 1, 2012.  After this, a $20 late fee will include and the registration will only be accepted if there is room on a team.
There are NO REFUNDS of registration fees except for the 2 circumstances
1.      A doctor’s note stating that the player is unable to play (up to the season start)
2.    No open spot on the aged team the player is registering for
·        Medical Release Form are for tournaments
·        Risk Management Form are needed if you are coaching, helping the coach, helping      the club, or are in any way involved with the kids (other than your own), you MUST fill this out.  It is an online form that goes to the Eastern Pennsylvania Youth Soccer Association (EPYSA) that ensures the safety of the children.
·         Incident / Complaint Form is used when you have a valid complaint or to report an incident.  Please realize we are ALL volunteers.  If you do have a complaint, please also include your idea of a solution.
